  • Recommendation Letter
  • Certificate of Health
  • Self Declaration of Health
  • Confirmation
  • Certificate of enrollment issued by your home university.
  • Your most recent academic transcripts, issued by your home university.
  • Application for certificate of eligibility.
  • Four facial photos (3 x 4 cms / 1.2 x 1.5 inches).
  • A copy of the front page of your passport (when available).
