- Enrollment Certificate (English version)
- Latest Academic Transcript (English version)
- Copy of Passport-size Photo (3.5×4.5cm / plain background)
- Copy of Passport (Valid for at least 12 months from the date of entry to Taiwan)
- Medical Enduring Power of Attorney: Please download it from the online system
